Review on Sciatica due to Osteochondroma of Proximal Femur: A Case report

  • What are the main claims of the paper and how important are they?

    The title of the article is sciatica due to osteochondroma of proximal femur, a mechanical pain due to entrapment /compression of the sciatic nerve by the huge sized tumor osteochondroma of proximal femur is the main claim.This is true in the sense ,there was subjective improvement of the pain following surgical removal of the tumor.They are very important in the sense, although the lesion of osteochondroma solitary or multiple are usually asymptomatic.They become symptomatic only when there is complaint of pain,mechanical due to entrapment or compression of the passing by nerve as in this case or even irritation or block of the tendons/muscles acting over the adjacent joints. There may be pain due to inflammation like bursitis over the cartilaginous cap or malignant transformation of the benign lesion as clinically suspected by sudden increase in size of the lesion which stopped growing after skeletal maturity. Thus the presentation of pain in osteochondroma which is usually asymptomatic becomes symptomatic with complain of pain..Thus complaint of pain becomes important demanding investigation for treatment.

    These are not novel .Infact these complaints have been mentioned in the literature and the authors also have mentioned.

    The statistical percentage of sciatica due to osteochondroma in that site and also entrapment neuropathy or mechanical irritation of the passing nerve by osteochondroma at other sites like knee,wrist,shoulder by review of literature should have been mentioned in Introduction or Discussion


  • Are the claims properly placed in the context of the previous literature?

    The claims of (1)sciatica due to compression by  huge sized  tumor osteochondroma of proximal femur and (2)relief of pain following surgical excision of the tumor in toto ,have been properly placed in the context of previous literature


  • Do the results support the claims? If not, what other evidence is required?

    Yes,the results of relieve of pain (subjective) following surgical removal of the tumor do support the claims.


  • If a protocol is provided, for example for a randomized controlled trial, are there any important deviations from it? If so, have the authors explained adequately why the deviations occurred?

    Since it is a case report study,the question of randomized controlled trial does not fit in such situation.


  • Is the methodology valid? Does the paper offer enough details of its methodology that its experiments or its analyses could be reproduced?

    The methodology of the case presentation is valid.But the details of the matter is lacking specially in introduction and  discussion.

    The topic was sciatica due to osteochondroma of proximal femur.In discussion only 2 case reports have been mentioned to support the claim of osteochondroma compressing the sciatic nerve.There is discussion more on incidence of the osteochondroma at different sites rather than compression neuropathy in this presentation.


  • Would any other experiments or additional information improve the paper? How much better would the paper be if this extra work was done, and how difficult would such work be to do, or to provide?

    I expect little more additional information like statistical percentage of incidence of compression of  nerve entrapment at that site as well as other sites like wrist,elbow,shoulder,knee.These additional information will definitely make this presentation more detail and complete.


  • Is this paper outstanding in its discipline? (For example, would you like to see this work presented in a seminar at your hospital or university? Do you feel these results need to be incorporated in your next general lecture on the subject?) If yes, what makes it outstanding? If not, why not?

    Yes, definitely I congratulate the authors for such an illustrative presentation.This is worth presenting in clinical meeting like seminar or case presentation in a conference.

    I do recommend this presentation for acceptance for publication with revision with additional information as suggested in above paras.
