    Pre op Preparation and hypotensive anaesthesia in endoscopic sinus surgery is of paramount importance as claimed in this article and it is true.Eventhough the endoscope gives a better illumination a bloody field greatly slows down the surgery and is a potential threat and may lead to complications.

    I think that this article comes from a combined effort of ent surgeon and anaesthetist which is novel.This topic of bleeding during endoscopic surgery and hypotensive anaesthesia is repeatedly discussed in everyother sinus surgery workshops and conferences.The claim in this article about use of specific agents and methods will have to be tried by other surgeons to formulate their protocol for edoscopic sinus surgery.

    They can take a certain number of similar cases and divide between two arms ,one with hypotensive anaesthesia and other without and analyse a set of parameters from the outcome like comfort during surgery,time taken,safety profile ,disease clearance etc and present it as a study.
