Other Comments:
The author opines that the death was due to coronary insufficiency simply because the left coronay ostia was narrowed. It is very difficult to come to such a conclusion based on naked eye examination.
Histological evidence in support of the opinion would be better.
Discussion should be in more details discussing how all other possibilities of sudden death were excluded.

Yes- this would be a great seminar in a hospital.
The author states that sudden death is often the first and only symptom of an advanced occlusive coronary arteriosclerosis. The case report presents the case of a 50 year old male. The results of the autopsy are also presnted and are very relevant.
In the discussion section, the author states that sudden death may be caused by stenosis of ostia of the coronary arteries. This is a result of syphilitic inflammation of the aorta. The thickening of the aorta is usually concentric.
