Thanks to Prof. Leticia Santos for reviewing my article. I agree that the data is not enough to show statistical significance making the report weak. That’s why I have presented it as a hypothesis and not as a research article.

    The article can be considered as a letter to the editor or to some extent as short communication.

    Some questions are evolved

    1- These observations were detected in 1981. Now we are in 2010. This led us to ask why were these observations published once after this date 1981.

    2- As indicated by the author, it is well described by peviuos publications that stimulation of vagus nerve, the main nerve of parasympathetic nervous system, causes stimulation of insulin secretions from beta cell and thus hypoglycemic effects, i.e. there is an inverse propotion between vagal activity and serum glucose levels. The authors did not mention publications and references which elucidate this relations?

    3- The author mentioned that some patients admitted in the ward with peptic ulcer disease had their blood sugar levels above normal. What is the percent of incidence with regard to the total number. What is the level of serum glucose in these patients; is it mild or sevre. I think in my opinion, the author must include the data.

    3- The author mentioned that the blood sugar levels in these patients came down to be within normal limits with anticholinergics containing hyoscinein. This evidence strongly support the hypothesis suggested by the author.

    4- The authors also mentioned that not all patients with peptic ulcer have high blood glucose levels because of several aetiology of the disease. Thus, inclusion of data analysis is essential.

    5- If the author include data obtained, the paper will be strong and confimiable.

Thanks to Dr. Osama Ahmed for reviewing my article. He has mentioned that he can suggest brief additions or amendments or an introductory statement that will increase the value of this paper for an international audience. I welcome him to do so.
The responses to his pertinent questions are as follows:
1- As mentioned I made these observations as a student-intern. I could not advance the work; neither did I find a platform like Webmedcentral to express my views. But all along I had wanted to publish it, so that any interested person having the facilities to do advance work could do so. Though the data are old and not large enough to reach a conclusion, but are enough to generate interest for future research.

2- I have put the reference (4. Best CH and Taylor NB Eds. The Physiological Basis of Medical Practice. The Williams and Wilkins Company, Baltimore, Maryland, 1966:1104.)

3- Total number of admissions was not noted, because I was not preparing to write an article. I noted down data of the men showing hyperglycemia, because it interested me. I observed ten men (no women, as I was working in male ward) having hyperglycemia.

N.B. Treatment was twice daily oral tablets of Hyoscine butylbromide 10 mg. for one week. Some patients left earlier and their blood sugar was not checked.
