Prof. Herchline’s opinion that my proposed “correct” answers are wrong is a possibility that I have always carefully considered. Moreover, I am sure that many other authoritative Scholars will share his firm belief. But, I think that the wrongness of each answer should be stated expressing specific criticisms and not an overall undocumented disapproval. As a matter of fact, Evolutionary Medicine – in particular the application of mismatch concept - is a drastic criticism of current medicine: it is a rule - which I have experienced several times - that even very skilful Scholars hardly accept or understand concepts and implications that overthrow many deep-rooted convictions.

I do not wish to comment questions and considerations which Dr. Libertini has made in this article. The worthiness of this article is to outline the lack of general culture and information on evolutionary medicine among Italian physicians. The course of study of medicine in Italy does not include evolutionary medicine, unlike other Western nations. I am certain that a comprehensive approach to the study of evolutionary biology would help students of medicine and physicians to understand the nature of disease much better. Disease is usually the result of a close relationship between an organism and its environment, including lifestyle, social and cultural background, as well as other factors.
