    This is a good case report on a relatively common condition in developing countries. The need for accurate diagnosis of pelvic-peritoneal tuberculosis without invasive procedures cannot be over emphasized. Unfortunately, this is often not the case as most ultrasonographic reports would suggest an ovarian malignancy. More so, most health facilities do not have infrastructure to do ascitic fluid Adenosine deaminase (ADA) and PCR assessment.

     

    However, the grammar used in the case report had some shortcomings which did not mar the understanding of the report. Also, the authors used two references to highlight the high sensitivity and specificity of ADA in the detection of pelvic-peritoneal tuberculosis but stated “several studies” which could be misleading to the casual reader.

    I have seen some similar cases in the past but diagnosis were only made at laparotomy at which one would see miliary seedlings of caseation necrosis, typical of Tuberculosis and Ascitic fluid AAFB would be positive. Some of these women had regular menstrustion which would not make you think of pelvic-peritoneal tuberculosis. This needs to be borne in mind too.
