    In this paper the author observed that Ubiquinone reach better results than Ubiquinol, as an oral supplementation of Coenzyme Q-10 in an elderly patient with high exercise aptitude.  
    The author recognized that “the results from a single-subject case report are definitively inconclusive”; however it deserves the conclusion that he also stated: “To determine whether or not performance is associated with exogenous Ubiquinone or Ubiquinol, more research is required…”
    It is an obvious claim for scientific community: the way to know the veracity of this single result, demand the planning of a controlled clinical trial.

    I am not working on Human trials still feel interested to read that how people are strongly working and giving strong results which are really fact that most of the drugs which we are taking are not at all needed but due to recommendation by the doctor or influenced by the mr or a long chain of advertisement......really this kind of work is needed....than may be one day we will find that generic drugs are best and not comparable with modern ones...

    We are trying th conclude and extrapolate from the results of this endurance training /supplementation all within 4 minutes of performance difference. If there is a culmulative effect of Ubiquinol / Ubiquinone on mitochondrial function, it should have been clearly mentioned in the article. The results are disparate and minor metabolic differences or age factors do not seem to strenghten the hypothesis. The need for a 90 days trial period becomes questionable.

     

    Change of "study" to "case report" is merely cosmetic and adds no insight.

     

    There is little weight to conduct formal controlled strudies.

     

    There is a heavy bias in the hypothesis.

Dear Dr. Gupta, There are 34 esteemed scholars who have reviewed this "Inconclusive" Case Report, none were discourteous nor poorly informed regarding the enormously complex mechanisms involved required to increase an endogenous hydrogen-carrier substance with a hydrogen-defcient substance in mitochondrion in place of and by an exogenous donor. Here are the last 2-sentences in the conclusion paragraph, in case you may have overlooked them: "Though this subject's best performance was recorded while taking Ubiquinone is surprising, a single-subject case report is inconclusive. More research is required to determine if exogenous Ubiquinol or Ubiquinone may hinder or enhance endurance running performance in males and females of all ages." Dear Dr. Herchline, I appreciate your review-comments, in that getting a passable "5" rating may be rare from you for a case report. I think you might be a good future case-study subject for placebo/double-blind research of ubiquinone/ubiquinol. In that manufacturer's research of both ubiquinone and ubiquinol contradict each other, because both argue they either out-perform or equal each other in steady-state blood serum levels post-dose. The important idea this case report asks is do steady state blood serum levels reflect distal cellular deposit? I asked this question because all the research I read prior to doing over 6-months of time trials on the same course did not turn out in favor of the Ubiquinol, but rather Ubiquinone. To be abruptly honest with you, this completely surprised me. Please let me know if you would be interested in being a "Test Subject" if we do manage to attain the funding for future research (I am not planning on doing it alone or without funds; it is too much work alone).     Coenzyme Q-10 in combination with other drugs are likely to be effective for improvement in general well being,  in prevention or management of heart diseases, diabetes and other metabolic disorders.

    This is an interesting case report where the author who is himself the subject has studied the effects of Coenzyme Q-10 on endurance runner's performance.

    This study has been well planned, executed and needs to be appreciated.

    This study has raised an important question regarding the benefits of Ubiquinone loading protocol.

    As this is carried out on a single subject, the reported effects have to be confirmrd on larger population. And also the gender variations,dosage specification, any other physiological changes can be studied.

Dear Dr. Aswini Dutt. R, I completely agree! As a Case Report can at most beg the question, we know that raising serum CoQ10 may or may not reach cells responding with increase energy output. This is especially true during exercise, when we have already confirmed that CoQ10 circulating levels are depleted -50% or more depending on age and total energy expense. Dear Dr. Constantino Ledesma-Montes, Thank you for posting your review of this Case Report. I agree with your final comments. The gain above washout-base without consuming oral dose of 2.28% performance seems very low as you indicated. However, the most recent USA National cycling time trial results show only a 2% difference between the Gold medal winner and the Bronze medal 3rd place finisher. In Track running events the differences are a fraction of a percent. With many athletes using harmful banned substances as reported by International and National Olympic organizations, would it not be a better choice by amateur and professional athletes to use a substance that is not harmful to health and also benefits the rate of energy metabolism for performance? Of course, my results are not confirmed unless and until a larger number of subjects are tested in controls using placebo-double-blind protocol.

    You are reporting an interesting case about a previously unexplored effect of coenzime Q10. I'm concerned with the magnitude of the effect. I doesn't seem that a ±2% variation in performance with treatments is significant enough to warrant further research.

As you likely know, oral dose CoQ10 is poorly absorbed, and as my paper states, food sources are not high in terms of amounts. Once diet has provided fat-soluble nutrients with either Ubiquinol or Ubiquinone, the amount effecting steady state blood serum transition to cells is estimated at between 2-8%. The main source of hydrogen molecule-exchanging Ubiquinol =Ubiquinone, is our body manufactured version, thus diet is not a big donor source, except some of the nutrients that come from foods are recruited to assist cell production of CoQ10. If an oral dose could raise steady state blood serum levels as both manufacturers of Ubiquinol or Ubiquinone claim, then how much, which one or both, would increase energy production during profound energy spent during exercise? If only 2% is gained in performance time to finish in a running race, that may be the difference between winning a gold medal and no medal at all. I agree it is not in terms of science method protocol a statistical significant finding, but to the athlete running the race, it is very important finding.     This is a very interesting, meticulously written article. However, I do have some points to make regarding the structure and overall readability and organization of the manuscript. (Note that this is me attempting to help turn a fascinating scientific study into a well-written and excellently presented article).

    In the introduction, there is quite a lot of repetition (esp. concerning the absorbance of Ubiquinone and Ubiquinol). There is a list, followed by paragraphs comparing the two - in some places this can sound redundant. The order followed in the introduction, beginning with comparing the two forms and THEN mentioning the functions of Coenzyme Q10 endogenously seems inappropriate. Starting broader by talking about CQ10 first (functions, nature, etc) and then it's forms and the differences between them would make it much more readable.

    The terms ''Not-healthy'' and ''healthy'' are inappropriate to describe high and low-fat foods respectively - they are not synonymous.

    The first sentence in the Methods section should be included in the introduction. A justification (or an attempt at one) for specifically selecting a 14-day washout period would also improve this section.

    The discussion should be before the conclusion, and more emphasis should be placed on comparing the results with other similar studies (in animals or humans) in the discussion. The discussion should be based on sentences such as ''Because aging and exercise create a deficiency ... etc'' rather than simply state facts about CoQ10 as has been done in the beginning of the discussion - most of this information would be better placed in the introduction. Otherwise, correlates between these facts and the results of the study should be considered.

  • Other Comments: I am basing my comments mainly from my experience in the field of medicine.  I am not sure if other discipline may have differing view.  When one reports a case, it represents among others; a perceived novel entity, a rare disorder that may not be rare after all or an unusual presentation of a common entity.  Nevertheless, they all share a common theme, that is a naturally occuring phenomenon or at the most an unintended consequence of our action.   The report is submitted, in the context that perhaps others may have also experience a similar situation or at least be alert of it.  Perhaps in due time, a pattern is revealed. In this report, a specific act was delibrately inserted to evaluate a response.   This is an interventional study, involving consumption of either Ubiquinol or Ubiquinin or none at all.  It looks like an attempt to do a cross over study.  When an interventional study is carried out, then the rules governing such a study has to be applied.  When studies are carried out without adhering to proper research methodology and statistical rules, we end up with misintrepetation, fallacies and worse still uninterpretable data. A single subject study do not tell us anything.   >> Each phase of the study was pretty long, 90 days.  The best-timed run for phase 1 was in April and phase 2 was in August, 2011.  The author has not detailed the exact period for each phase.    I believe in a temperate climate, dietary habits changes with the seasons.  In the tuskegee study, there was almsot 30% drop in food consumption by weight in winter compared to spring, mostly due to dropped in consumption of animal products (55%).( Robert T. Dirks and Nancy Duran, 2001)  The author of this study (auth herein) has mentioned in the introduction that co-enzyme A level is influenced by consumption of fatty food.  Has the auth standardized the dietary intact during all three phases of study.  It would have been better if serum Q10 level was also quantified during the study.  This would have given a better correlation between performance in each phases and the level of Q10, if that is what the author wish to study.  This brings us to the next issue. >> The auth has not specified the research question.  This is a fundamental step in any hypothesis driven empirical research.  The research methodology design depends on the research question.   >> The auth had presented his result as best of 12 runs in phase 1, best of 8 in phase 2 and a single run in phase 3.  The agreed method of presenting such data would be central tendencies and its associated intervals.  All these run-time values may be just a difference by chance, which we could statistically evaluate by repeated measure ANOVA.  .     >> The subject was not blinded.  The subject has complete control on the conduct of his run with full knowledge of which supplement he has taken.  This is a recipe for bias.   >> There should be a washout period between phase 1 and 2. >> There are other issues as well but it becomes irrelevant when the core issues is at fault. The auth stressed that no conclusion is made for this study.  However the displayed result indirectly imply to some that maybe a shorter time was achieved during phase 2.  This is totally erronous.  Besides every manuscript must have a take home message, i.e some form of conclusion.  Otherwise it should not be written in the first place, as others could not benefit from the write up. In the end, I believe this manuscript has not provided us any additional knowledge compared to where it began.  WMC has an open policy of accepting and publishing all manuscript.  The responsibility now rest on the writers to evaluate his or her own paper  before submitting.    It is my personal opinion that authors should refrain from publishing interventional study of single subject.     P.S. my comments has been purely in an academic context.     This article has been extensively revised in response to reviews of the previous version. It reads much better now, although the organization is still somewhat awkward (Conclusion before discussion, some parts of the discussion would be more appropriate in the introduction and visa versa).

     

    The author now clearly describes the findings as interesting without drawing any conclusions. I agree that further research with more subjects (and blinding of investigator/subject) would be worthwhile.

Dr. Puliyel, Thank you for your comments and critical review. Blinding would have been performed including times listed from each 90-day period. The facts are simply that had we done blinding with separated timed runs, then we would have been comparing N=12 to N=8, which would have shown not numerical assessment of timed performances. Because I was author and subject, I was limited to being able (age71) of attaining resting heart levels low enough <10% resting values to be timed within guidelines of a runner rested well enough to perform at optimal level. I thought that if you knew this background of why one oral dose product could only be tested by time trial performance 8-times and the other product 12-times you might understand why running this open-label case report actually presents no bias that I could detect. Point is we could not do a controlled timed performance on an runner unless that runner's resting HR indicated complete recovery. I thought you deserved an explanation because your criticisms were concerns I agree for they were mine as well. Thank you for your review, Bill Misner PhD

    You did well to change from case study to report. Your last setence in Conclusion section says it all. That sentence already takes care of the limitations inherent in the report.     In this single case study, the authors reported the effect of Ubiquinone on performance of 71 years old male subject. This observation is of great interest but still some questions remain to be answered before going to large clinical studies.

    This article is well designed and written however two points should be taken into consideration: 1) the introduction is too long and is not focused on previous studies. 2) the discussion appear particularly weak and seems a repetition of the Introduction.

    Single-case studies are always welcome before conducting “real” ones (clinical trials, follow-up studies, on similar), because we learn not only about possible/desirable outcomes, but moreover about all possible mistakes, obstacles, and difficulties to avoid later.  This study is of significant importance considering that it brings new perspectives on researching Ubiquinol or Ubiquinone and anaerobic exercising, but I feel that the author encountered some difficulties, he needs to avoid in a later project. I think that the issues arose during the study are of greater importance than technical ones with its presentation.

    1. Shorter abstract is preferred.  Like “This case study evaluated the running performance of a 71-year male runner while taking Ubiquinol, or Ubiquinone, each for 90-days, or taking neither supplement following a 14-day washout period. The subject best performance time on the same course while taking Ubiquinone was 2.28% faster than his time on the same course after a 14-day washout period no-supplement dose. The subject best performance time on the same course while taking Ubiquinol was –1.16% slower than his time on the same course after a 14-day washout period no-supplement dose.” This is the key note!
    2. In the intro, details of the previous studies that evaluated the drug and physical exercise should be given. However, what was written in the discussion is actually the content of the intro.
    3. The aims of the study are missing and what the hypothesis is. This need to be clearly stated, whereas it will be the aim of the further research.
    4. It needs to be clearly stated why including a 71-year old male. Why not including a younger individual? We need to know the entire health status of that individual as well.
    5. You need to state in the discussion what are the implications of these findings and how they relate to the context of this drug. You need to give what should be the next step.

    Clinical trials on single subjects are helpful before actually proceeding with case control studies on a larger samples. In relation to the present study, I suggest the following:

    1. A placebo drug could have been tried on an age matched control after assessing the fitness.

    2. Double blinding would have erradicated bias.

    3. The placebo could have been tried on the same case subject between the phases, with wash out periods between each phase.

    4. A case control study with larger sample size is welcome as the results are promising.

Dr. Rao, Thank you for presenting an excellent review with comments I agree completely. I will add the following thoughts that I have been reluctant to share until now, since I conclude that your review best states changes that will improve further study in this arenae. ADDENDUM FOR REVIEWERS Prior to the test trials, I prepared by reading the science written by the scientists working for both manufacturers. Each argued with each other to a stalemate. The Kaneka rationale matched some science-method reports connected with aging, while Best Formulations rationale stated that Ubiqiunone was more stable than Ubiquinol, and that stability for the exogenous form favored Ubiquinone and that they each were involved interchangeably and needed for trading off hydrogen molecules during the energy cycle. Both manufacturers stated that their product maintained steady state blood serum levels better than the other. Who was I to believe? I therefore hypothesized that if I took 400mg/day of either Ubiquinol or Ubiquinone during 90-days of timed runs over a common course that I had trained on to attain familiarity and fitness, that one or both versions would exceed base performance, determined after a 14-day washout period (half-life of CoQ10 is 36-72 hours) in a resting subject) and since I was training during the washout period 60-90 minutes average daily, the washout 14-days would easily washout all previously dosed CoQ10. The methods section of this paper did not detail that prior to each timed trial, resting heart rate the day prior to each test was between 51-56 bpm. (resting low HR =51bpm). If the resting heart rate was not within this range, recovery being incomplete, no timed run performance was determined. Max Heart Rate for a 71-y male based on the Karvonnen formula, is 149 bpm. Heart rate at the end of each timed performance ranged between 156-164 (104%-110% max heart rate).This is a clear indication that on the day a timed performance was tested, the subject was adequately rested and at finish had expended himself to the full. I have no connection with or competing interest in either form of product and am persuaded that bias was absent. Nevertheless, double blind and placebo would and should be employed to address the issue of bias. The first paper proceeding this one's title was changed from "Case Study" to "Case Report." Physicians write Case Reports regularly reporting an anomaly in detail based on their objective and many times subjective observations, without placebo, nocebo, or blinded protocol structure. A "Case Report" several of the reviewers stated precedes or justifies tightening double-blinded and placebo protocols to control for the sake of responsibly collecting data for determining conclusions. This "Case Report" is an open label experiment in which the author was the subject who strictly maintained the same low fat whole plant food dietary menu during the year prior to and the year of these 21-timed performances collected during this 194-day period. In my view, this "Case Report," though inconclusive as based on sound science method principles, presents specific information observed and reported by an one elderly fit 71-year subject for recruiting a larger contingent of adult male and female fit subjects to measure and evaluate the effects of Ubiquinone or Ubiquinol in double-blind-placebo on timed endurance performance.     The information preceding the Methods section would be better titled "Introduction."

    It is difficult to draw conclusions from the way this data data was presented. The statistical tests carried out to determine significance were not stated.
