I think this reviewer should revise your review.

It is clearly the reviewer has not drunk from the supplied references, mainly from this author on specialized journals with high impact index (1-2).

Otherwise, he would have realized that it is not an isolated case (that is not a case report) , this author has already published many similar clinical cases in the International Journal of Cardiology as well as British Medical Journal.

There are a great difference between different conscience levels: drowsiness, stupor and coma (Glasgow, from 3 up to 15 points in adults) that is also unknown by the reviewer (sorry!)

Nothing to do lithium blood levels with bipolar disorders. In fact, lithium treatment in psychiatric disorders remains speculative treatment as should know the reviewer.
In fact, any patients had psychiatric pathology.

In any case, thank you very much and to say to reviewer that I also know a lot about ECG.

By the way, the reviewer could explain what he /she has called the Q-Q interval related with lenght Q-T interval according his literature?
Would be very interesting.
